Background

This paper presents the findings of a 2025 evaluation of the Royal College of Midwives (RCM)’s perinatal mental health (PMH) roadmap that was launched in autumn 2023. The PMH roadmap was designed to influence and guide the provision of PMH services, and strengthen the role of midwives.

Aims

To determine the PMH roadmap’s impact, identify next steps, and ensure the RCM’s continued meaningful engagement with key stakeholders and commitment to prioritise PMH.

Methods

A review of internal and external documents, and feedback from over 65 stakeholders across the UK via surveys and qualitative interviews.

Findings

While the PMH roadmap has successfully raised awareness of the need for specialist PMH midwives and has been a valuable tool for strategic influence in some areas, its overall impact is hampered by a lack of implementation. Key barriers to effective PMH service provision include inadequate funding and resources, a lack of senior management buy-in, fragmented care systems, and significant variations in specialist midwife roles.

Recommendations

That the RCM PMH roadmap be updated to focus on three key themes:
• Infrastructure
• Workforce
• Education and practice development.
